The objective of this study was to evaluate exogenous drug poisonings in Brazil from 2008 to 2022. Methodology: An ecological time series study was carried out, evaluating notifications of exogenous drug poisonings. The annual number was retrieved from the Notifiable Diseases Information System (SINAN) and normalized to every 100,000 residents. The significance level was set at 5%. Results: In the last 15 years, 522,657 confirmed cases of exogenous drug poisoning were reported. The person-year incidence during this period was approximately 257 cases per 100,000 residents. The majority of confirmed cases occurred in people between 20 and 39 years old (43.6%) and females (72.5%). The main circumstance was attempted suicide (71.1%), with a single acute exposure (67.9%). The temporal trend was increasing in all macro-regions and in Brazil (p <0.05). When comparing incidences, it was observed that the North and Northeast macro-regions were below the national estimate, while the South was above (p <0.05). Final considerations: There was a significant increase in confirmed cases of exogenous drug poisoning in Brazil between 2008 and 2022, in addition to macro-regional disparities.
