Implementation of the National Early Warning Score in nursing practice: incorporation of a technology for clinical surveillance in a federal hospital

Abstract

Objective: To analyze the process of incorporating the National Early Warning Score into the nursing team's practice in a medical ward of a federal hospital. Method: A field study with a qualitative and descriptive approach, conducted through semi-structured interviews, systematic observation, and document analysis. The interviews were analyzed using IRAMUTEQ software through Descending Hierarchical Classification. Results: The implementation occurred progressively, with adjustments throughout the study period. Lexical analysis revealed operational and organizational challenges related to the incorporation of the technology, as well as recognition of the scale as an instrument to support clinical surveillance, decision-making, and patient safety. At the end of the analyzed period, 68.6% of the forms showed adequate completion, indicating partial implementation of the tool. Conclusion: The scale was recognized as a relevant technology for the early detection of clinical deterioration; however, its consolidation depends on continuous training, institutional monitoring, and adaptation to care routines. The incorporation of healthcare technologies in public hospitals involves not only the availability of instruments, but also the reorganization of work processes, continuing education, and the strengthening of the patient safety culture.

    Nurse, PhD in Science, and postdoctoral researcher, CAPES fellow (PIPD), affiliated with the Graduate Program in Translational Medicine at the Escola Paulista de Medicina, Universidade Federal de São Paulo (EPM/UNIFESP), Brazil. She has academic and clinical experience in Pediatrics and Neonatology, patient safety, health technologies, aeromedical transport, and health management, with experience in high-complexity hospital settings, including service as a Nursing Officer in the Brazilian Air Force. She has completed international academic and institutional technical visits at the University of Oviedo (Spain) and the Escola Superior de Enfermagem do Porto (Portugal). She develops extension and hospital volunteer activities, especially in the Florecer Project at the Hospital Universitário Gaffrée e Guinle (HUGG), integrating science, humanized care, and social commitment, and is a member of research groups affiliated with UNIFESP and UNIRIO.
