Abstract: Objective: To report the construction process, execution and experiences of telemonitoring by nurses and residents of the Family Health Multiprofessional Residency Program Primary Health Care, to patients suspected or confirmed for COVID-19, in a municipality located in the interior of Bahia. Brief description of the experience: This is a study carried out in two Family Health Units, in an urban area, between March 2020 and December 2021, in a municipality with medium vulnerability. The monitoring of patients that was previously defined centrally by the Municipal Management, became decentralized, attributed to the Basic Health Network, on June 1, 2020, when the telephone devices were sent to the PHC health services. Conclusions: The experiences reaffirm the importance of readjusting and qualifying Primary Health Care to promote telemonitoring and mitigate some of the impacts caused by the pandemic to the service, so that there would be no discontinuity and worsening of the conditions of users in isolation and allow the opening of a channel that maximized engagement. This tool is shown to be an efficient and resolute strategy for monitoring the population and containing cases of COVID-19, since it optimized the search for information, provided patient safety, enhanced care coordination, ensured adequate directions, reduced exposure between health professionals and patients, reduced the movement of people within the service and also enabled savings in the use of personal protective equipment.Downloads
